Twin Ports Walleye Association Tournament Release Form

A. Liability Release, we the undersigned hereby release the Twin Ports Walleye Association, committee, volunteers and sponsors from any and all liabilities costs, claims, damages and demands related to personal injury, death or property damage incurred while competing in the Twin Ports Walleye Association Tournament. We further acknowledge that before competing in the Twin Ports Walleye Association Tournament the committee has a right to deny a team or participant in competing in the Tournament.

B. Insurance, We agree to maintain $100,000.00 of liability insurance on any boat operated during the Twin Ports Walleye Association Tournament.

C. We agree to submit to a polygraph / stress test at any time during the course of time covered by the Tournament Committee. We acknowledge that failure to take or pass test by either contestant will result in disqualification with no recourse. All prize money and awards will be withheld pending results of tests.

D. Photo release, we agree that any photographs or other promotional material taken during the Twin Ports Walleye Association Tournament is the property of the Twin Ports Walleye Association and may be used by the Twin Ports Walleye Association, it’s sponsors, or the media without royalties or prior consent.

E. Agreement, we agree to abide by all rules and regulations set forth by the Twin Ports Walleye Association Tournament and the state where Tournament is held. We agree that the Tournament Committee’s decision will be final in all matters, we signify by our signatures below that we have read, understand, and agree to abide by all rules and stipulations set forth by the Twin Ports Walleye Association.
